Subjective, Objective, Assessment, Plan — medical-origin note format.
A SOAP note separates the client's self-report (Subjective) from clinician observation (Objective), followed by Assessment and Plan. Common in medical settings; used in behavioral health when payer or organizational requirements demand it.
