ASAM-defined tiers from outpatient to medically managed inpatient.
ASAM defines Levels 0.5, 1, 2.1/2.5, 3.1/3.3/3.5/3.7, and 4 — from early intervention to medically managed inpatient. LOC drives payer authorization; SUD notes must justify current LOC.
